Juventus want to reunite Jorginho with Maurizio Sarri next season and plan to offer Miralem Pjanic in return for the Chelsea midfielder, claims Sky Sports.
The Old Lady have been heavily linked with a move for Jorginho since Sarri left Chelsea to become Juventus head coach over the summer.
Sarri turned Jorginho into the star of his Napoli side and took the Italy international with him to Chelsea in a deal worth more than £50m two years ago.
Jorginho has struggled to make the same impact in the Premier League and now Juventus intend to make a serious play for the midfielder at the end of the season.
It is unclear whether Frank Lampard would consider Jorginho's departure as the 28-year-old has been a first-team regular during his first year in charge of the Blues.
However, Juventus have a plan to prise Jorginho away from Stamford Bridge by offering Pjanic in exchange as a direct replacement.
Pjanic has spent the last four years at Juventus but his future at the club is in serious doubt, having also been heavily linked with a move to Barcelona in recent weeks.
The Bosnia international is, however, two years older than Jorginho and Chelsea tend to stay clear of signing players aged 30 or over.
Pjanic has made 22 league appearances for Juventus this season and returned a combined total of five goals and assists.
In the side for his progressive passing, however, he ranks fourth in the division for most passes per game (66.3).
